Communicating Social Determinants of Health: A Role for Coding

Recognition of the impact of social determinants of health (SDOH) on patient outcomes is growing, as is the desire to incorporate SDOH factors (e.g., neighborhood and housing, income, education, food security, social support, transportation, and access to technology) into care plans.

To be accounted for in clinical settings and transformational to whole-person clinical care, SDOH information needs to be coded and interoperable across the health and human services ecosystem using standard coded terminologies and standards for exchange like HL7 Fast Healthcare Interoperability Resources (FHIR®).

As the steward of the Current Procedural Terminology (CPT®) code set and a member of the Gravity Project (an SDOH collaborative), the American Medical Association (AMA) is a driver in the effort to create and standardize the coding for social risks and needs.
